(Those under 18 need to be accompanied by a parent or guardian)

Please note:

• Only available to established HHCMG primary care patients
• Only providing the Pfizer Bivalent booster to ages 12+ in the PC offices. (For those needing the initial series, please visit one of our drive-up locations, or a local pharmacy.)
• Certain HHCMG pediatric offices will provide the Pfizer primary series and booster, if applicable, to children under 11 years old.

• Those under 18 need to be accompanied by a parent or guardian. If you do not have proxy access to your child(ren)’s MyChartPLUS account, the parent or legal guardian of the child being vaccinated will need to complete a proxy form at the clinic site. Forms will be available, but you can save time by downloading the proxy form and taking the completed form with you to the clinic. DO NOT SCHEDULE A PEDIATRIC VACCINE APPOINTMENT ON YOUR OWN MYCHARTPLUS ACCOUNT.
